FDA Lifts Longstanding Warning on Hormone Therapy: What It Means for Women in Canada

Hormone replacement therapy (HRT) is back in the spotlight. The U.S. Food and Drug Administration (FDA) has removed the “black box” warning that for more than 20 years cautioned women about serious health risks such as heart attack, stroke, and breast cancer. This label stemmed from early 2000s research on older women using outdated hormone formulations — studies that have since been reinterpreted in light of newer data.

Recent evidence shows that these risks do not apply equally to all women. For those who are younger or within 10 years of menopause, and especially when using low-dose or non-oral forms of therapy, HRT can be both safe and effective. Medical organizations, including the American College of Obstetricians and Gynecologists, now encourage individualized care rather than a “one-size-fits-all” approach.

For us in Canada, this is a timely reminder that menopause care continues to evolve. While Health Canada’s labelling remains unchanged, many Canadian clinicians align with the latest research and international guidelines. The key message: HRT is not automatically risky or automatically right — it’s a personal decision based on your age, health, and unique needs.

What This Means for You

  • Personalized care first: Risk and benefit vary by age, timing, and overall health.
  • Best started early: Women who begin HRT before age 60 or within 10 years of menopause often see the greatest benefits.
  • Safer formulations: Transdermal (patch, gel) and low-dose therapies are linked with lower risks.
  • Canadian context: Though our regulations haven’t shifted yet, the trend toward individualized treatment is the same.
  • Next step: Book a consultation with our Naturopathic Doctor – Dr. Rehana Kassam, to explore whether HRT or other therapies could support your menopause journey.
